We offer two convenient ways to pre-register for a hospital
admission.
Telephone
You may call 289-7162 Monday through Friday between the
hours of 9am and 5pm.
Online
Please read the following instructions before pre-registering
with us online.
Our online registration form is two pages and should take
an average of 5 to 7 minutes to complete. You should register
online no earlier than 30 days in advance and no later than
two weeks prior to your hospital admission. Once submitted,
it will take approximately three working days for your registration
to be internally processed. Upon arrival to the hospital,
we will need your signature to finalize the admission process.
